 I will conduct thorough assessments by using the strength based approach and making sure i maintain a person centred approach to understand your loved ones needs and develop personalised care plans.  This is acheived by completing a full and comprehensive Care Act assessment, which includes support planning.   This assessment can be used to determine the correct level of either care at home, or to take the next steps, should your loved one not be able to remain at home safely.  

I will provide a full and comprehensive report, which will outline  the care and support that your loved one will need.  My service includes all time taken to do the assessment, write up and travel to meet your loved one and your family.  Furthermore, I will, with consent, contact health professionals, such as the GP, as health information forms a vital part of this assessment. 

*Sourcing care at home, or a suitable placement, should your loved one not be able to remain at home, is not included in this service.
